BBH Coaching Evaluation
Welcome to BBH Coaching Evaluation Tool
Brain Body Health is a coaching program to understand, assess your risks for dementia, and provide treatment options and lifestyle changes.
STEP ONE:
Begin by completing a 15-minute free ZOOM evaluation to determine the appropriateness of BBH for you. If you are having memory challenges a family member needs to be present. At this meeting, it will be determined if you are a candidate for BBH coaching. If so, you will be given appropriate resources and an appointment date. Appointments can be in person or on ZOOM.
STEP TWO:
Before your first appointment: Start by answering the 14 questions below in a document on your computer. Forward this document to bettylacymd@gmail.com two weeks before our appointment.
Reason for visit: Your concerns about memory, thinking, completing tasks, confusion, navigational challenges, or others.
Medical History: Include: current medications, recent medical evaluation and laboratory work, vital signs; especially blood pressure, and dental history.
Hospitalizations, surgery, general anesthesia.
History of exposure to chemicals, mold.
Menopause/Andropause (Male menopause) symptoms.
Mental/Emotional Health History: Therapy, hospitalizations, medications.
Cognitive testing history; last MOCA or MMSE.
Habits: alcohol, cannabis, drug usage, food challenges, social media, describe your daily activities.
Family history of dementia, heart disease, other?
SLEDSSSS:
Sleep: history of sleep apnea/amount per night/napping/challenges/snoring/tests?
Learning: What new things are you learning in your life? Significant medical/LAB tests, APOE4 test.
Exercise: type, amount, frequency?
Diet: eating times, your daily habits, fasting, challenges.
Sensory Health: recent vision and hearing testing/genitourinary/sexual health.
Socialization: what does your support look like in your community?
Stress reduction: what practices do you use? How would you describe the benefits and challenges of aging?
Spirituality/End of Life planning/Advanced Directive complete?
Actions toward Prevention: List Vaccinations, DEXA SCAN, Mammogram, PSA, Colonoscopy, other…
Detailed notes of your BBH evaluation will be provided via email. Discuss these with your primary care provider (PCP). Request these notes be placed in your medical file. Some tests will be requested by your PCP

Complete the McCance Brain Score.
First Appointment: (Allow 90 minutes)
Review history above.
Discuss cognitive impairment with client and support person.
Provide Prescription for blood work.
Schedule return appointment.
Discuss your BBH evaluation with (PCP).
Second Appointment: (Allow 90 minutes)
Discuss any new developments.
Review findings.
Assess risks for dementia via metrics from laboratory, history, genetics (APOE4), MOCA or MMSE cognitive tests, radiological testing, and SLEDSSSS questions.
Follow up appointments will be determined on the results of the evaluation.
Third Appointment: (Allow 90 minutes)
Discuss concerns and questions.
Discuss End of Life Concerns.
Complete Advanced Directive.
Decide on follow-up appointments: This will be decided on an individual basis and my availability.
After Care:
1) Discuss findings with your primary care provider and bring copy of your BBH notes. Some tests will be required by your primary care provider such as an MRI of brain with Neuroquantitative analysis.
Service Fees: $150/ 45 minutes; $300 for 90 minutes. Fees will be pro-rated on time basis.
NOTE: Collaboration with your primary provider;
Tests such as Calcium Score: (Heart CT), MRI with Quantitative Analysis, Neurocognitive Testing, and Blood testing for amyloid, tau, Nfl, markers for Alzheimer’s Disease will need to be ordered by your PCP. These tests and the ongoing visits with BBH will involve collaboration between you, your family member, or health care proxy/POA for health care, myself, and your PCP will be of utmost importance to our team work.
